Description CASAC-T, CASAC, LMSW, LMHC l Supporting clients in-house & in the community Arms Acres is a licensed facility providing inpatient and outpatient treatment for those suffering from addiction to alcohol or drugs. Sign-on bonus eligible To provide comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; referent relations; discharge planning; and intervention with client support systems, including social service programs and managed care. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties as required. Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures. Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames. Documents patient progress and own interventions in case record. Need to remain within time frames of regulatory requirements. Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director. Performs utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities. Attend and/or facilitate Family program. Responds to emergency situations in the facility by following appropriate procedures for ensuring patient safety during emergency procedures. Maintains professional competence in discipline and treatment specialty areas through continuing education and supervision. Monday through Friday Monday, Tuesday and Friday -11am-7:30pm Wednesday and Thursday 9am-5:30pm Requirements QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
