Description Case Manager- Outpatient Clinic - Sign on bonus of $4,000 for new employees. Arms Acres is looking for a qualified, passionate Case Manager to join our clinical team at our Outpatient clinic in Queens, NY. Arms Acres is a licensed outpatient clinic providing treatment for those suffering from addiction to alcohol or drugs. As a Case Manager with Arms Acres you will function as part of an interdisciplinary team and provide comprehensive psychosocial assessment and diagnostic services to patients in an outpatient setting. While providing process oriented & didactic group counseling services and individual, family or crisis counseling to a caseload. Requirements Case Manager I: Associates Degree in Chemical Dependency or related field with 1 year of applicable experience, Current CASAC-T or CASAC Certification, CPR Certification required within 6 months of hire Case Manager II: Bachelor’s Degree in related field, with one year related experience, CASAC or Qualified Health Professional (QHP per OASAS 800 Regulations), CPR Certification required within 6 months of hire Case Manager III: Master’s Degree in related field, Current LMHC or LMSW with one year of experience, CASAC or Qualified Health Professional (QHP per OASAS 800 Regulations), CPR Certification required within 6 months of hire We offer competitive wages, benefits, and a pension plan in a supportive working environment.
