Job Title: Case Manager – Supportive Housing
Location: Bronx, NY
Job Type: Full-Time | Temp-to-Perm
Pay: $22.00 – $25.00 per hour (based on experience and qualifications)

CASE MANAGER QUALIFICATIONS:

• Bachelor’s degree in Human Services or related field + 2 years’ case management experience
• 1+ year of scatter site case management experience preferred
• Experience working with individuals living with HIV/AIDS, substance use disorders, serious mental illness, or homelessness

ABOUT THE ROLE:

Atlas Search is seeking a dedicated and compassionate Case Manager to join a supportive housing program in the Bronx. This is a temp-to-perm opportunity within an amazing non-profit known for its strong leadership, supportive environment, and clear opportunities for career growth. The Case Manager will oversee a caseload of 22 clients, providing comprehensive services to help individuals achieve stability, retain housing, and increase their capacity for independent living.

CASE MANAGER RESPONSIBILITIES MAY INCLUDE:

• Conduct comprehensive assessments and develop individualized service plans with short- and long-term goals
• Maintain regular contact with clients (minimum: 1 home visit/month and 1 contact/week)
• Connect clients with primary medical care, mental health, substance use, housing, vocational, and other necessary services
• Provide crisis intervention, housing retention support, and tenant rights education
• Assist clients with moving into permanent housing, including coordination and transportation when needed
• Collaborate with internal teams and external providers
• Maintain accurate case documentation in compliance with agency and funder requirements
• Participate in supervision, training sessions, and team meetings
