Wage: $23.00 to $25.00 per hour Schedule: Monday to Friday 9am to 5pm (35 hours) Location: Bronx TASC Office - 1020 Grand Concourse North Professional Wing in the Bronx Primary Purpose of Job: The Case Manager provides screening, treatment planning, case management, and court reporting of eligible clients for the Treatment Alternatives for Safer Communities (TASC) Programs. Principal Duties & Responsibilities: Act as liaison to the Courts. Provide screening services for the more routine and less complex cases within the judicial system for clients involved with alcohol or drug abuse and/or who present evidence of mental illness in order to ascertain appropriate treatment as an alternative to incarceration. Under supervision, follows and implements a comprehensive screening to effectuate referrals to a mental health, drug abuse, or alcoholism facility and accompanies and/or arranges transportation for clients to the appropriate facility when necessary. Assist clients in applying for and receiving public and private benefits. Maintain case documentation available for review by supervision at any time, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with program requirements. Maintain a caseload of clients and monitor their participation in the treatment program through case management appointments, reviewing progress reports, and conferencing cases with treatment personnel. Prepare and present written and oral status reports to the criminal justice system in a timely manner regarding client's progress. Investigate and recommend supportive services for clients. Conduct drug testing on clients of the Program as needed. Perform all other relevant duties as determined by Supervision and EAC NETWORK Administration Knowledge, Skills & Abilities Required: Associate’s degree in a helping profession required and at least 2 years of prior experience working in intervention programs with criminal justice involved participants. Bachelor’s degree preferred Experience conducting health education groups and or cognitive behavioral interventions preferred Knowledge of treatment programs and local criminal justice and health systems preferred. Good communication skills, both oral and written; Ability to work cooperatively with treatment and criminal justice systems. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
