CMHA Toronto’s ideal candidate will have an undergraduate degree or diploma in social services or a related field. The candidate will bring a minimum of three (3) years’ experience in direct service delivery to adults with serious and complex mental health issues, including those with concurrent disorders. Experience in community mental health considered an asset.
The Candidate will have a sound knowledge of forensic mental health, the criminal justice system, psychosocial rehabilitation, recovery principles and supportive housing. The candidate will bring strong assessment, service planning and crisis prevention/intervention skills. The candidate will also be able to work with diverse communities and demonstrate an anti-oppression approach to practice. The candidate will have excellent communication, listening and interpersonal skills and have the ability to work within a multidisciplinary team as well as independently. The candidate will also be required to have a valid Ontario Driver’s License, access to a vehicle and insurance coverage.
SALARY: $54,124 - $62,733

Responsibilities:
THE ROLE AND OPPORTUNITY
The Canadian Mental Health Association is looking for a Transitional Case Manager for our Transitional Rehabilitation Housing Program 1 (TRHP 1). This position is a result of an initiative designed to facilitate the placement and support of forensic patients into transitional and supportive housing within a 24/7 staffed CMHA operated building, with the goal being rehabilitation and community reintegration. The TRHP 1 team is partnered with the Centre for Addiction and Mental Health’s (CAMH) Complex Care and Recovery Program (CCRP) and Forensic Outpatient Department (FOPs), and provides service to 12 forensic clients in collaboration with the forensic mental health system and other community partners. All clients have been found Not Criminally Responsible (NCR) and are under the purview of the Ontario Review Board (ORB). Transitional Case Managers hold forensic designate status with CAMH and work to balance clients’ recovery goals with risk management and community safety.

RESPONSIBILITIES
Reporting to the Manager of Forensic Services, in collaboration with the TRHP 1 Clinical Lead, the Transitional Case Manager will work with the interdisciplinary team to support their clients. They will carry a caseload of 6 clients, with this number fluctuating based on program needs. They are required to participate in team meetings and provide regular updates to the manager and lead. The Transitional Case Manager will act as a liaison between related CMHA Programs and CAMH’s CCRP and FOPs, as well as other community service providers.
The aim of the Transitional Case Manager will be to assist in the development of a service care plan for their clients and linkage to appropriate community services. This will involve orientation to the program, agency, and community as well as supporting clients to understand and adhere to the condition of their ORB dispositions. There will be a requirement of providing on-site and community-based services and interventions, including support with ADLs, maintaining structured day time activities, group facilitation, social recreation programming, psycho-education, and structured relapse prevention. This role will involve risk assessment/management, crisis prevention/intervention, monitoring, evaluation and follow-up. The Transitional Case Manager will also work to maintain trusting, respectful and collaborative working relationships with their clients and colleagues, both external and internal. There may also be some coordination of site maintenance at the Weston Road site, where the program is based out of (currently - site location to change). This is a site-based position (North York).
