CASE MANAGER – SHORT STAY CRISIS BEDS PROGRAM (FULL-TIME CONTRACT – 3 MONTHS) ---------------------------------------- THE ROLE AND OPPORTUNITY The Canadian Mental Health Association Toronto offers intensive case management services as part of the continuum of mental health services and supports for people with mental illness. We are currently seeking a full-time contract Case Manager for our Short Stay Crisis Beds program. The Short Stay Crisis Beds program provides 24-hour on-site residential support and short-term case management services to individuals living with mental illness who have been involved with the criminal justice system. The Case Manager will offer onsite support to clients as well as community-based services and interventions. Join us in providing high quality mental health and addictions services, advocating for a better mental health system, and providing education and mental health supports to clients across Toronto.   RESPONSIBILITIES * Program outreach and assessment * Orientation to program, agency and community * Client-centered engagement, service provision and interventions * Crisis prevention and intervention * Skill development and goal planning  * Providing resources, rights information and advocacy  * Maintaining a trusting, respectful and collaborative working relationship  * Monitoring, evaluation and follow-up * Accurate, timely documentation * Some coordination of site maintenance duties may be required SKILLS YOU BRING The successful candidate will possess the following: * Minimum undergraduate degree/diploma in Social Work/Social Science or related field * A minimum of 3 years full-time experience in direct service delivery to adults with psychiatric disabilities  * Strong assessment, service planning and crisis intervention skills * Work experience and knowledge of concurrent disorders, homelessness, recovery principles and the criminal justice system as it relates to mental health  * Strong organizational and time management skills * Ability to work collaboratively in a team environment * Excellent communication, listening, and interpersonal skills * Ability to demonstrate an anti-oppression approach to practice A valid Ontario driver’s license is required. Successful applicant must have access to a car and insurance for workplace use. The candidate will also have strong written and oral communication skills and understanding of recovery principles as they relate to individuals with mental illness.
