We are currently seeking a Full Time Case Manager for our Care Management for Adults & JACC program, Hackensack, NJ. Job Duties: The Case Manager is responsible for the development and management of a service plan for clients that includes direct counseling services. Formulate and implement individualized service plans. Responsible for establishing continuity of care and assuring appropriate quality services.
