RELEVANT EXPERIENCE:

Preferred: 2-4 years of relevant experience
Strong blend of interpersonal, organizational skills, with great communication, empathy, problem-solving, and critical thinking skills.
Knowledgeable in computer and technology skills.
Ability to drive mobile van with support staff
Minimum: 1-2 years of relevant experience
Computer literacy and data entry knowledge required
Be able to advocate and coordinate resources for individuals within underserved communities.

Responsibilities
  • Conduct intake and assessments to create timely individual service plans with measurable goals and objectives, provide short and long-term case management services to individuals and families.
  • Knowledgeable about community resources and advocate for services with community partners.
  • Works collaboratively with therapists to support clients and have a flexible schedule to meet the long-term and short-goal needs of the clients, which may include evening or weekend support.
  • Maintain up-to-date records, files, databases, and routine reports. Must meet all required productivity standards.
  • Solicit, log, sort, store any donations or deliveries for the program, and engage in community outreach and driving through mobile van efforts.
  • Adhere to agency and programmatic policies and procedures, professional standards of conduct, and any relevant licensing and accrediting bodies for performing job expectations.
  • Participate in community meetings and represent the agency regarding services and timely reporting of program needs
