As the Case Manager, you will play a pivotal role in individuals’ lives, making a significant impact through:

  • Completes the timely and accurate documentation in the Uniform Case Record, including but not limited to: referral intake packets and related intake information, referrals and related correspondences, entitlement documentation, logbooks, and other areas as assigned
  • Reviews all intake information for new referrals and performs initial screening assessments for intake criteria and makes recommendations regarding acceptance or denial
  • Develops and implements the ISP and then every 6 months thereafter, reports outcomes and makes needed changes
  • Addresses the monthly progress achieved against specific milestones and targets related to housing and referral for services
  • Organizes and facilitates support groups and social and recreational activities including but not limited to issues relating to daily living skills, treatment, housing permanency concerns, and entitlement eligibility
  • Processes statistical information, progress reports, and written discharge information, using computer-based software. Compiles and submits data to the Director of Support Services, for the preparation of monthly, quarterly, and annual reports
  • Conducts case conferences with other case managers, Director of SRO Housing, staff (as necessary), Department of Social Services, and other involved treatment providers

Case Manager Minimum Salary: $ 20.00
Case Manager Maximum Salary: $ 23.00
Case Manager Schedule: Mon-Fri 10:30am-6:30pm (35 Hours)

Qualifications:

  • Associates Degree in Social Work
  • Bi-lingual in Spanish preferred
  • Valid driver’s license, proof of current car insurance and registration and access to a reliable, working vehicle
