JOB SUMMARY

We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will have a strong background in nursing and case management, with experience in hospital settings. QUALIFICATIONS

• Previous hospital experience is highly desirable.
• A nursing degree or equivalent qualification is required; active nursing license preferred.
• Knowledge of acute care practices and case management principles is essential.
• Familiarity with medical terminology to effectively communicate within the healthcare environment.
• Experience in hospice care is a plus, particularly for those working with terminally ill patients.

• Assess patients’ needs and develop individualized care plans in collaboration with healthcare providers.
