Description The Case Manager will coordinate and deliver integrated services to children and families within the UCS Children’s Integrated Services program. This role focuses on early intervention and mental health support for children from birth to age 5 and their families, helping them navigate social, emotional, and behavioral challenges. ESSENTIAL DUTIES AND RESPONSIBILITIES Assessment and Planning: Conduct comprehensive assessments of children and families, developing individualized service plans (One Plans) tailored to their unique needs. Service Coordination: Facilitate and monitor services across various providers, ensuring families have access to the necessary resources and support systems. Family Support: Provide education and guidance to families, helping them build advocacy skills and navigate community services. Collaboration: Work with multidisciplinary teams, including educators, healthcare providers, and community organizations, to ensure effective service delivery. Meetings and Reporting: Attend weekly, monthly, and quarterly meetings for the CIS program to report progress and track data for grant stakeholders. Data Tracking: Collect and maintain data related to client outcomes and service delivery to provide comprehensive reports to stakeholders. In-Home and Classroom Support: Work within homes and classrooms to provide support to caregivers and educators, helping them accommodate and manage children's behaviors effectively. Follow-Up and Evaluation: Regularly assess client progress, adjusting service plans as needed to respond to changing family dynamics. Documentation: Maintain accurate and timely records of client interactions and services in compliance with UCS contract requirements. Community Outreach: Engage in outreach activities to raise awareness about UCS services, promoting access for families in need. JOB QUALIFICATIONS: High school diploma required. Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred. 2-4 years of relevant experience working with preschool-aged children, such as in roles like preschool teachers or aides, or in case management with children and families. Valid driver’s license and auto insurance. LOCATION & HOURS Work is performed both in an office setting and within the community. Core hours are 8:30 a.m. to 4:30 p.m. Occasional after hours scheduling as needed.
