Case Manager: We are seeking a full time BA level candidate to provide case management, outreach, supportive counseling and to coordinate healthcare for children and their family in our Randolph catchment area. Working as part of the team, you will be offered weekly clinical supervision and support in a team-based learning environment.
Essential functions include:
- Providing case management, care coordination, supportive counseling & problem solving to clients in our Child and Family program
- Documenting clinical services in an electronic medical record.
- Gathering & completing paperwork to open charts, document progress and discharge when appropriate.
- Providing outreach & support to clients in the community and their homes.

Pay: $24.00 - $25.00 per hour
