    JOB SUMMARY  Through clinical expertise and knowledge of current case management, utilization and discharge planning criteria, this position is responsible for supervising staff that ensure patients receive timely, efficient and appropriate services in the most suitable level of care setting at CCAD.   POSITION DIMENSIONS Reports directly to Manager, Case Management Reports indirectly to   Head Count Managed   Number of Direct Reports   Number of Indirect Reports   Financial Accountability      ROLE INTERACTS DIRECTLY WITH TITLE/DEPARTMENT FREQUENCY TYPE OF INTERACTION Various patient care areas  As needed Patient care   KEY DECISIONS THE ROLE MAKES AND THE EXTENT OF THE IMPACT OF THE DECISION     PRIMARY JOB DUTIES AND RESPONSIBILITIES       CLINICAL / PROFESSIONAL KNOWLEDGE   * Develops and implements operational plan for Case Management, including Utilization Review and Social Work * Utilizes Case Management software application to analyze trends and prepare reports * Presents data to various stakeholders in a concise and articulate manner * Identifies trends and makes recommendations for improvement * Promotes interdepartmental collaboration  through effective communication * Manages staff through ongoing performance evaluation, coaching and counseling, and with clinical visibility * Facilitates effective group process in Nursing and intra-disciplinary groups through sound application of team dynamics * Implements shared decision making processes * Manages multiple priorities concurrently and assists staff in effective  time management * Liaises with payers and regulators to ensure documentation requirements are met * Advocates on behalf of the patient to protect the patient’s health, safety and rights * Responsible for completing charge capture education and in-service training for their Institute within CCAD. When charges are not captured automatically the nurse will complete within Epic in real time, Perioperative Services within 24 business hours         ROLE LEVEL ACCOUNTABILITIES   * Adheres to CCAD’s standards as they appear in the Code of Conduct and Conflict of Interest policies   In view of the evolving needs and opportunities within CCAD, this position may be required to perform other duties as assigned and reporting relationships may vary.   KEY OUTPUT / RESULTS FROM THE ROLE (KPI’S) KEY PERFORMANCE INDICATOR MEASURE TARGET See Specific unit-based KPIs       PHYSICAL DEMANDS AND WORK ENVIRONMENT Typical physical demands include: a high degree of manual dexterity to sufficiently perform computer functions on a keyboard, produce materials on a PC and operate basic office equipment, normal or corrected vision and hearing, extensive sitting, standing and/or walking, mobility sufficient to visit with departments or employees throughout CCAD and lift up to 30 lbs.   ORGANIZATION-WIDE COMPETENCY ASSESSMENT REQUIREMENTS All employees will embrace the CCAD mission, vision and values and be responsible for adhering to the core values of the institution, including:  Patient’s First, Collaboration, Mutual Respect, Quality, Patient Safety, Integrity, Cultural Sensitivity and Compassion. Customer Service Orientation includes attitude, behavior, interpersonal skill and problem solving that enable an employee to respond to internal and external customer needs and expectations in a positive manner.  Adaptability including teamwork, flexibility needed to fulfill job responsibilities, adapting to changes in work environment and accepting supervisory feedback.  Efficiency and Effectiveness includes quantity and quality of desired work, as well as organization skills necessary to perform successfully.  Essential Job Requirements includes adherence to all relevant policies, procedures and guidelines affecting the work environment, as well as maintenance of required competencies and communication skills.  Managerial Responsibilities includes overall accountability for assigned work group relative to operational goals, personnel requirements and budgetary constraints.   * Assume responsibility of own safety and health and will abide by CCAD Occupational Health and Safety policies and procedures as per OSHAD and any other relevant requirements.   QUALIFICATION & EXPERIENCE REQUIREMENTS   ESSENTIAL PREFERRED Education * Bachelor’s degree in Nursing, or equivalent qualification (minimum three (3) year duration) from an accredited program, is required   Experience * Minimum of six (6) years clinical experience in an appropriate and relevant healthcare setting post qualification is required * Minimum of two (2) years as a Senior Charge Nurse (S.C.N.) or minimum three (3) years in a Charge Nurse position is required * International accreditation experience is preferred * Indication of progressive leadership, management and clinical positions is preferred * Knowledge of pre and post acute venues of care * Demonstrated ability to appropriately interact with physicians, nurses, patients and their families   Certification and Licensure * Bachelor’s degree in Nursing, or equivalent qualification (minimum three (3) year duration) from an accredited program, is required * Valid and current national license is required * BLS is required * Certificate of post graduate diploma in Nursing/Healthcare/Management/Administration field is preferred Professional Membership * N/A     Job Specific Skills and Abilities * Excellent interpersonal skills * Proficient   in the use of PCs and spreadsheets * Strong organizational and time management skills * Ability to speak and write in English is required * Effective oral and written communication skill   * Ability to speak and write in both English and Arabic is preferred
