We’re seeking a Case Manager to guide patients and families through the rehabilitation journey, ensuring seamless communication, effective care planning, and smooth transitions. How you’ll contribute: Lead interdisciplinary care coordination and discharge planning Identify barriers to progress and help develop solutions with the care team Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines Facilitate weekly team conferences and family meetings Communicate patient progress and discharge plans to caregivers Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities Support additional departmental needs as assigned Qualifications and requirements: Active licensure as an LVN, RN, Social Worker, Respiratory Therapist, PT, OT, or SLP, required Bachelor's Degree; preferred 2+ years of case management or social work experience in an inpatient setting (rehab experience strongly preferred) Strong communication, organization, and collaboration skills Proficiency in Microsoft Office Case Management or Rehab Nursing certification (CCM, ACM, RN BC, ARN); preferred Why join us We believe that investing in our employees is the first step to providing excellent patient care.
