The Case Manager will be responsible for developing service plans designed to ensure stable, permanent housing and monitoring of our clients for up to six months.  Incumbent acts as an advocate and liaison between the client and community service providers to ensure appropriate utilization of resources in meeting the needs of the client. 1st shift 8:00-5:00 pm 2nd shift 11:30am- 8:00 pm. Education and/or verifiable work experience with at-risk populations is required (homeless, veterans, behavioral health, drug/alcohol addiction, employment, youth, residential).
