POSITION SUMMARY   Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.   ESSENTIAL FUNCTIONS * Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.  * Case manage total care of the patient by coordinating with team members/physicians, supervising C N A activities, and participating in staff and Interdisciplinary Team meetings. * Complete daily paperwork and charting, thoroughly and accurately according to Policy. * Participate in on-call rotation. * May assist with admissions   QUALIFICATIONS * Registered Nurse in the state of Texas   * Hold a valid driver’s license and have a dependable means of transportation   * 1 year of nursing experience   * Case Management experience (hospice)   * 2+ years of experience in hospice case management strongly preferred   * Proficient skill using a computer is required; Microsoft office suite experience is preferred.    BENEFITS * Competitive Pay * Generous Paid Time Off Programs * Company provided Life Insurance, Short- and Long-Term Disability  * Medical, Dental, Vision  * Flexible Spending Account and Health Savings Account  * Employee Assistance Program * Retirement Savings Plan  * Mileage reimbursement for work-related travel Reasonable Accommodations Statement  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
