Craig’s Doors seeks a reliable, trustworthy, independent second-shift case manager with a strong interest in working with vulnerable and complex populations. This is a role with high independence and high responsibility. The best candidates for this role are extremely self-motivated, task-oriented, and able to manage their time effectively to respond to guest needs while staying on top of follow-up tasks and data reporting requirements. This role will be performed during second shift hours (3-11PM), 4 days a week, to fill in the gaps that can be missed outside of regular business hours and serve a need for this community that does not otherwise exist. One day a week will be dedicated to training and other needs to round out the full-time schedule.
Job Summary: The Case Manager at Craig’s Doors works under the supervision of the Director of Case Management to provide comprehensive case management services to guests accessing our program locations in Amherst and Hadley. The Case Manager conducts assessments, develops individualized service plans, connects guests to resources, and provides ongoing support to help them achieve stability and self-sufficiency.

QUALIFICATIONS

  • Bi-lingual English/Spanish preferred.
  • Bachelor’s degree in Social Work, Psychology, Public Health, or a related field.
  • Valid Driver’s License, ability to reliably commute to work, and transport guests when necessary.
  • Must have regular access to a reliable vehicle as work requires frequent travel between the sites in Amherst and Hadley, MA.

OR

  • High school diploma/GED and a minimum of 1 year of experience in case management or social services. *
  • Knowledge of trauma-informed care and case management principles.
  • Strong interpersonal, communication, and problem-solving skills.
  • Ability to work effectively both independently and as part of a team.
  • Proficiency in organizational and time management skills.
  • Valid Driver’s License, ability to reliably commute to work, and transport guests when necessary.
  • Must have regular access to a reliable vehicle as work requires frequent travel between sites in Amherst and Hadley MA.

Responsibilities
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered.
  • Provide orientation to program guidelines and responsibilities.
  • Conduct comprehensive evaluation and assessment of client needs, strengths, limitations, barriers, and goals.
  • Utilize motivational interviewing techniques to empower guests and promote positive change.
  • Work with individuals to develop realistic service plans that identify issues and map a course toward their resolution. Provide long-term support as they work towards stabilization. Service plans may include assistance with housing, employment, income, education, health care, mental health, substance use disorders, disability support, domestic violence, legal and credit issues, or a combination of the above.
  • Continually assess, refine, and develop service plans that will improve the lives of our guests while working with organization leadership.
  • Provide orientation to program guidelines and responsibilities for new guests
  • Help maintain a clean organized workspace.
  • Monday - Friday (9am - 5pm)

Housing Support:

  • Assist clients with housing search, including completing applications for public housing, vouchers, private housing, and supportive housing.
  • Provide coaching and skill-building to enhance housing readiness, including financial management and training programs.

Resource Connection

  • Assist guests with housing searches and applications for services such as veteran’s benefits, social security, food stamps, cash and fuel assistance, etc.
  • Connect guests to community resources including food pantries, schools, medical facilities, governmental offices, and public transportation.
  • If necessary, provide transportation to appointments in Craig’s Doors van.

Relationship Building and Conflict Resolution

  • Provide or arrange for training on tenant’s rights, financial success, and other activities that support long-term sustainability.
  • Initiate new community partnerships and support clients in establishing new social relationships.
  • Advocate for clients rights and needs with landlords, emergency responders, and other agencies.
  • Provide crisis intervention to support clients, bringing in outside agencies when necessary.
  • Create response plans in the event of health crises.

Documentation and Reporting

  • Maintain accurate, comprehensive, and timely logs, case notes, and client records.
  • Document crisis incidents for Craig’s Doors records and state reporting.
  • Collect, enter, and maintain all necessary statistical data required by Craig’s Doors and our funders.

Professionalism

  • Adhere to professional standards and protocols outlined by Craig’s Doors.
  • Utilize effective communication skills with guests, staff members, departments, and organizational leaders.
  • Maintain and implement organizational and time management skills to effectively manage caseload.
  • Utilize problem-solving skills and multitasking abilities to address guest needs and overcome challenges.
