Description SCHEDULE: 40 hours, Monday – Friday, 8:00 a.m. - 4:30 p.m. Pays $ 23.48 - $26.30 per hour DOE (Salary ranges provided are based on relevant experience and skill set) LOCATION: 35 Creighton Street Jamaica Plain, MA The Permanent Housing Case Manager is a non-essential position in the event of an emergency SUMMARY OF POSITION: Permanent Housing provides comprehensive and intensive case management support to homeless and chronically homeless individuals in housing, utilizing the Housing First Model to promote long-term stability and the end of homelessness in Boston. The Permanent Housing Case Manager works as part of an intensive case management team serving tenants who may be experiencing untreated mental illness, active drug and alcohol addiction, and other significant health challenges. The case manager uses a harm reduction and trauma-informed approach to working with participants, utilizing Stages of Change Theory and Motivational Interviewing techniques. The case manager is responsible for a specific caseload of tenants and for collaborating with other Permanent Housing staff on program-wide efforts to stabilize and promote the individual growth of all congregate housing tenants. Additional case management responsibilities include, but are not limited to assisting tenants with all required program documentation; assisting tenants with obtaining benefits, health care, educational opportunities, employment opportunities, and legal aid; supporting the development of tenants’ activities of daily living skills and creating individual service plans with tenants, and maintaining all necessary documentation. QUALIFICATIONS: The Permanent Housing Case Manager must demonstrate a clear commitment to the population we serve and must be capable of working as part of a team. Requirements EDUCATION/TRAINING: REQUIRED: High School Diploma or GED Effective written communication skills· Proficient in Microsoft Office products PREFERRED: Bilingual – Spanish/English · Undergraduate degree in a related field (i.e., social work, psychology, education, nursing) KNOWLEDGE/EXPERIENCE: REQUIRED: Experience working with individuals facing substance abuse, mental illness, trauma, homelessness, and/or other complex disorders. ·Strong commitment to learning and utilizing Harm Reduction and Motivational Interviewing skills to promote improved behavioral health among tenants. Knowledge of effective crisis intervention techniques and the ability to respond appropriately in crises. Experience collaborating as part of a team. PREFERRED: Experience in Housing First programs supporting chronically homeless individuals. Familiarity with local caregiving and advocacy resources for those facing substance abuse, mental health issues, trauma, head injuries, medical conditions, physical disabilities, relationship violence, and justice system involvement. Understanding of government benefit systems and service providers in the Boston area. Experience with HMIS
