Case Manager – Downton Day Services Center Reports to: Associate Director of Homeless Services Location: Washington DC Salary: $55,000 - $58,000 Hours: 8:00a – 4:30p   Position Purpose At Pathways to Housing DC, we believe housing is a human right and that recovery begins with trust, dignity, and connection. The Case Manager for the Downtown Day Services Center (DDSC) plays a vital role in supporting adults experiencing homelessness through compassionate, person-centered care. This position provides case management, therapeutic support, advocacy, and community-based services to individuals living with serious mental illness, substance use disorders, and other complex health challenges. The ideal candidate is someone who values relationship-building, meets people where they are, and is passionate about helping individuals navigate pathways toward stability, wellness, and housing. What You Will Do * Provide outreach, case management, counseling, advocacy, and other needed services to clients seeking services in Center.  * Collaborate with client in the formulation of an Individual Recovery Plan (IRP) that is reviewed and modified with client on a regular basis; * Provide therapeutic counseling and support services to each client in a broad range of areas, including (but not limited to) housing, training & work opportunities, activities of daily living, health, wellness self-management, relapse prevention, self-help and empowerment, problem solving, family life and social relationships and integrated treatment for substance abuse; * Coordinate and monitor referrals to community services, and advocate client’s participation in them.
