Essential Duties & Responsibilities (includes but is not limited to the following): Meet with survivors, their children, and pets upon entry into the Emergency Shelter Program. Provide ongoing quality trauma-informed case management services to shelter residents including survivors, their children, and pets. Develop goal plans that regularly assess the needs, safety, and well-being of the survivor as well as those of their children/pets. Assist survivors with exploring/establishing/maintaining a continuum of services including but not limited to: employment/ eligible income; budgeting skills; school enrollment/childcare; pet care including veterinarian care; connections to medical, mental health, and substance use disorder resources as applicable; education for basic living/ parenting skills, locating/securing permanent housing. etc. Provide referrals and assistance with connecting to all available Safe Berks services and community resources. Provide D&A education groups and assessments for Safe Berks shelter residents. Initiate referrals for school age children to the BCIU under the McKinney-Vento Act, when appropriate. Support survivors and their children with safety planning, identifying natural and community supports, and ongoing goal planning. Coordinate exit plans for survivors, their children, and pets leaving the emergency shelter program. Explore community resources and develop/strengthen community partnerships with other social service agencies. Maintain accurate, timely, and comprehensive case notes and electronic records in accordance with agency standards. Attend and actively participate in individual/group supervision, staff meetings, and required trainings. Crosstrain with Crisis Hotline & Intake Advocates and Residential Services Advocates to ensure coordinated service delivery as well as assist with residential/crisis department coverage. All other duties as determined by department director. To perform this job successfully, an individual must be able to successfully perform each of the essential duties. Educational & Experience Requirements: 1 – 3 years’ experience working in the Human Services field or in an advocacy-based role. Prior experience providing case management to underserved communities required Associate’s degree in Human Services or a related field preferred; relevant education may be substituted with equivalent professional experience. High school diploma or equivalent degree required Experience or familiarity with trauma-informed care, substance use education, and survivor advocacy strongly preferred. Ability to work collaboratively, maintain confidentiality, and manage multiple priorities in a fast-paced environment.
