Program Description FTC - STARR is a collaborative partnership program designed to meet one of the most critical community care needs in San Francisco – providing outpatient case management, and wraparound services for adults with co-occurring substance use disorder who have had contact with the criminal justice system. We provide intake, assessment, and triage assisting individuals who are diverted or discharged from jail to immediately access SUD treatment. FTC - STARR provide a Case Management services to work with families referred to FTC at 100% FTE. Hours worked will be reported by Contractor via monthly or quarterly invoice. The Case Manager will be experienced in providing outreach, engagement, referral, and linkage services. In collaboration with other members of the FTC team, the Stabilization Case Manager will be responsible for: serving as a member of the FTC team and Perinatal Stabilization Team (PST); outreaching to participants referred though PST or other FTC referral sources, providing assertive case management, skill-building, crisis intervention, and harm-reduction support to assigned FTC participants; working collaboratively with FTC partner agencies to provide wrap around support and to develop, with participant input, an effective Family Wellness. Care Plan; utilizing Motivational Interviewing, Cognitive Behavioral Therapy skills, and other evidence-based and evidence-informed practices suitable for diverse participants; supporting participants to access appropriate community-based resources, including accompanying participants to appointments and/or court dates; monitoring and tracking participants’ progress in achieving their goals and objectives; working collaboratively with FTC partner agencies and other community-based, social service agencies to help maintain stability, promote recovery and/or mental and health care support; participating in FTC case conferencing and status hearings, Child and Family Team meetings, and another participant- and family-focused collaborative meetings. Participate in FTC governance meetings, such as twice monthly Core Team meetings, grant management meetings, etc.; complying with requirements for documentation and reporting. adhering to SAMHSA data collection guidelines and requirements; adhering to laws regarding confidentiality and reporting requirements and maintain necessary compliance documentation. planning and facilitating skills groups in person and virtually as needed; other duties as required by the program. Job Description Case Managers provide integrated reentry and substance use support through individual interventions, including harm reduction, motivational interviewing, cognitive behavioral therapy, and targeted life skills. Services will include: · benefits enrollment; · linkage to primary care providers and medication support; · support accessing housing assessments and collaboration with on-site housing operators; · basic needs support (food, clothing, personal grooming items); and · referred client screening and clinical assessments. Case Manager will report client progress to the FTC team as clients appear on the calendar. The Case Manager will be responsible for data entry into SAMHSA’s SPARS system according to requirements of the Government Performance and Results Act (GPRA) of 1993. Case Manager will enter data reporting in a timely manner, including, but not limited to, referral date, exit date, exit status, and participation in services. Case Manager will provide client updates including treatment plan adherence for each court date for each client. The Case Manager shall attend case conference every day that an FTC client is on calendar and will attend court upon request of the Court so that the Court is not delayed in its reporting obligations. Minimum Requirements and Qualification High school Graduate (or GED) required. Bachelor’s degree in a Social Sciences field preferred (Sociology, Social Work or Psychology, Criminal Justice, etc.) Minimum one-year experience working with Justice Impacted Adults who are experiencing substance use disorders, mental illness, homelessness or related challenges. Interested in engaging in and connecting with a diverse, high-need community of adults Ability and willingness to provide non-judgmental encouragement to clients is essential Intermediate knowledge and experience working with computers, including word processing and data entry programs. Written communication skills in English adequate to draft basic progress reports and meet the documentation requirements of the STARR program. Ability to develop an Individual Intervention Plan, IIP for each client Ability to connect clients to services that relate to criminogenic needs and risk factors Able to effectively organize and present information in group settings Able to work effectively both independently and collaboratively.
