Description STATUS: Full-time (37.5 hours a week) ]HOURLY RATE: Up to $21.00 SUMMARY Lifespan is a trusted, mission-driven, regional nonprofit dedicated to helping older adults take on the challenges and opportunities of longer life. Lifespan’s Financial case managers holistically assess financial needs and assist older adults in managing their financial affairs and maintenance of personal independence within a fee-based service. This position is responsible for maintaining an average caseload between 30-35 clients and is responsible for general management of financial affairs including bill paying and helping plan and maintain long-term financial security of clients. The case manager is responsible for processing Social Security, Medicaid and other benefit related applications. The Case Manager is responsible for meeting with clients a minimum of two times annually and more frequently depending on the level of need. This is a community -based service and the Case Manager will meet with clients in a variety of settings including the individual’s residence, skilled nursing facility, hospital or financial institution. The case manager delivers services in a proactive rather than reactive framework. There are no after-hours or on-call responsibilities with this position and requests for hybrid work schedules are considered after successful completion of the probationary period. DUTIES AND RESPONSIBILITIES 1. Assess and investigate financial situation of clients. 2. Determine and plan appropriate actions that will assist client in becoming as financially secure as possible. 3. Provide full bill paying and budgeting services to ensure long-term security. 4. Assist with financial decision- making. 5. Provide referrals to other community resources assisting the client to remain as independent as possible. 6. Maintain and manage all records, case notes, billing, reports and metrics as required ensuring accuracy and compliance with agency departmental policies and procedures, including state and federal tax laws. 7. Advocate with financial, creditor and other institutions and agencies on behalf of client. 8. Appropriately bill clients for services rendered. 9. Participate in all Lifespan staff meetings, trainings, continuous process improvement and in the implementation of agency mission and vison 10.. Other duties as assigned by supervisor. Requirements QUALIFICATIONS EDUCATION: Associate in applied sciences required. Bachelor’s degree in business, Finance, Human Services or equivalent Preferred. EXPERIENCE: 2 years’ banking or financial services experience required. Must have strong financial acumen and a working knowledge of community resources. Expertise in MEDICAID and other benefit progress preferred. Prior experience working with and advocating for older adults preferred. PHYSICAL AND MENTAL REQUIREMENTS · 50% of the work is community based: must be able to meet with individuals in their home environment and in a variety of community settings including hospital and nursing home environments. · 50% of the work is performed at a desk or workstation · Must be able to meet with clients present in their homes and have a valid NYS Driver’s License. · Ability to drive tactical results to achieve strategic goals. · Must demonstrate Knowledge of the eligibility requirements and application processes of benefits and programs applicable to clients, including but not limited to SSI and Medicaid, MMLTC, VA, Medicare, SNT. · Must have attention to detail and, ability to multi-task. · A portion of the work is performed at a desk or at a personal computer station. · Strong organizational, verbal, written and telephone communication skills required. · Computer experience preferred (Word, Excel, EMS, Office 365, web research). · Comfort with Interfacing with all levels of personnel. · Ability to maintain composure under strict deadlines and difficult situations. · Ability to work in multi-cultural situations. · Normal daily walking and sitting. Frequent computer usage. · Some lifting (files, etc.)
