This position does require working in the community and performing home visits. - Bachelor’s degree in a human services field from an accredited institution required. - LLBSW, LBSW, LLMSW, LMSW, LMFT preferred. The Case Manager primary responsibilities include: Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness. In a residential or clinical setting, provide clinical and program consultation to direct care staff. Ensure case management compliance with agency standards and the practice model. Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community.
