Position Summary: The Case Manager supports the treatment team by coordinating care and services that align with each patient’s treatment goals. This role involves facilitating referrals, assisting with discharge planning, and connecting patients with community resources to promote long-term recovery. The Case Manager ensures all services meet licensing, ethical, and evidence-based standards. Key Responsibilities: Monitor, coordinate, and manage assigned patient cases and referrals. Collaborate with clinical staff to assess and address case management needs. Connect patients to essential services, including healthcare, housing, vocational, and social supports. Maintain communication with treatment teams, referral sources, and community partners for coordinated care. Provide education and support to patients and families, including participation in the HEART program. Facilitate family sessions and deliver patient education groups as needed. Requirements Qualifications: Bachelor’s degree in human services field required. Valid driver’s license and safe driving record required. Knowledge of substance use disorder (SUD) and mental health recovery programs. Familiarity with community resources and ability to assist patients in accessing them. Strong organizational, communication, and computer skills (including EHR proficiency). Ability to collaborate effectively within a multidisciplinary team. Culturally sensitive and able to work with diverse populations. If in recovery, must have sustained abstinence and demonstrate rehabilitation and stability.
