What You’ll Do: Support individuals who struggle with their mental health—including adults with severe and persistent mental illness and emotionally disturbed youth—in building skills, increasing independence, and working toward meaningful personal goals. Serve individuals with Severe and Persistent Mental Illness & Severely Emotionally Disturbed youth, as well as other assigned individuals requiring case management services. Collaborate with clients and families to develop and implement individualized treatment plans that address mental, emotional, and social needs. Travel throughout our 20-county service area to meet clients where they are—at home, in schools, or in the community (Center vehicle provided). Lead or assist in group activities that focus on building life skills, managing symptoms, and improving overall quality of life. Advocate for your clients, helping them navigate systems and access essential community resources such as housing, education, healthcare, and employment support. We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college. Great with people—strong communication and empathy are a must. Comfortable in a variety of human service settings. Able to pass drug screen & be subject to a random drug testing pool. Must have valid driver’s license.
Responsibilities
The Case Manager will support individuals struggling with mental health, including adults with severe and persistent mental illness and emotionally disturbed youth, in building skills and achieving personal goals.
