As a Housing Case Manager, you will provide case management services to individuals and families with a drug, alcohol and/or mental health disability residing in Berks Counseling Center’s various housing programs. This may involve transitional, permanent supportive, or mental health jail diversion. You will also… Complete a comprehensive assessment of clients’ skills, strengths, and needs Complete new client intakes and review of client’s rights and consent forms. Work cooperatively with clients to develop a unique service plan based on each client’s skills, strengths, and needs. Encourage, support, and assist clients in meeting support plan goals. Assist clients in locating resources and provide referrals to those resources. Assist clients in overcoming barriers to follow through with services such as needs for childcare or transportation Requirements To qualify for the position, you will need… Either a bachelor’s degree in the social services field Or a minimum of six (6) years of combination of education and experience beyond a high school diploma or GED. A valid PA Driver’s License and the ability to drive, travel, and provide community-oriented services. A willingness to work with integrated team members. To be able to work 7:30am – 4:00pm; some flexibility may be required to accommodate for client availability. Physical requirements: SEEING: Must be able to see well enough to read reports and use computer. HEARING: Must be able to hear well enough to communicate to clients, vendors, and coworkers. MOBILITY: Must have mobility to move about the agency. Must have mobility to move about the community. LIFTING/PULLING/PUSHING: Must be able to lift up to 10 pounds. OTHER: Must be able to write, type, and use technology. Preferred candidates will … Have experience working with individuals with co-occurring disorders preferred. Be bilingual - able to effectively communicate verbally, orally, or in writing in English and Spanish. Have knowledge of the 12 Step Program and Community Resources a plus. Have knowledge of or experience with trauma-informed approaches preferred.
