About Us Avivo’s Ending Homelessness division helps individuals and families move from homelessness to housing stability. We partner with county coordinated entry systems, tribal governments, and operate unique street outreach teams to connect people with shelter and permanent housing. We offer support in both scattered-site and site-based housing, including transitional recovery housing and Avivo Village—a low-barrier, culturally responsive shelter designed to support unsheltered individuals on their path to stable housing. Our work is grounded in harm reduction, housing first, and trauma-informed care with a commitment to culturally responsive, person-centered services. Your Role As a Case Manager in our scattered site housing programs, you’ll provide one-on-one support to individuals and families referred from Hennepin County shelters. You’ll help clients set goals, access community resources, and prepare for long-term housing success. Your role includes assisting clients in shelters and in the community as they transition into stable housing and beyond. Schedule: This is a 32/hr. a week full-time position paid for 40 hours. Key Responsibilities Include Maintain a caseload and meet regularly with clients (minimum once per month, plus 3+ outreach attempts). Build trusting relationships through creative engagement and consistent follow-up. Conduct needs and housing assessments; develop and monitor person-centered goal plans. Support clients in gathering documentation, accessing healthcare, and connecting to services. Identify housing options and coordinate with landlords to ensure housing stability. Provide crisis intervention, eviction prevention, and re-housing support when needed. Complete all required documentation, notes, and reports in a timely and accurate manner. Participate actively in team meetings, supervision, and staff development activities. Contribute to Avivo’s Undoing Racism and equity goals through ongoing learning and culturally responsive practices. Qualifications What You Offer High School Diploma or GED required. No experience required if you hold an Associate's degree or higher. At least 1 year of related experience if you do not hold a degree. Lived experience may count toward up to 1 year of experience. Ability to support individuals with serious mental illness and complex needs. Knowledge of community resources, trauma-informed practices, and housing systems. Strong interpersonal skills, commitment to anti-oppressive practices, and ability to work independently and collaboratively. Reliable transportation, valid driver’s license, and ability to travel locally.
