Work Location: San Rafael, CA Employment Details: Full Time, Non- Exempt, Monday - Friday during business hours Compensation: $27-30/hour. About the role: We are searching for a Case Manager I for our Casa Canal Permanent Supportive Housing Program! This is an exciting role to gain Case Manager experience with the unhoused population. The expected case load is up to about 20 participants. Since the participants are housed, the role is focused on providing them resources to the community, ensuring they are comfortable in their home and will also have the opportunity to lead workshops from creating a resume, to finances and more! What you’ll do: Intake and Assessment Screen prospective participants by verifying income and documentation, obtaining necessary consents, and reviewing eligibility with supervisors. Conduct comprehensive assessments (HMIS, VI-SPDAT, Housing Retention) to evaluate participant strengths and risks while providing full orientation on program services, rights, and responsibilities. Case Planning and Monitoring Conduct interviews and research to identify risks, stratify needs, and develop data-driven care plans. Establish participant-driven goals based on resource availability, consensus, and required level of care. Provide financial coaching—including budgeting, tax filing (EITC), and credit repair—to support long-term stability. Link participants to internal SHELTER, Inc. programs and external community resources, ensuring minor children are enrolled in school. Maintain a caseload of 20 households, facilitating case conferences and acting as a liaison between service partners and participants. Perform routine reassessments and proactively adjust care plans to improve outcomes in static or regressive situations. Manage case closures and exits—both voluntary and involuntary—ensuring all agency procedures and adverse action protocols are followed. Data and Documentation Keep all relevant service data up-to-date in HMIS system and maintain case files in accordance with applicable guidelines, completing data entry immediately or no later than within 24 hours of service interaction. What you’ll bring: 1+ years' experience working with low income and/or homeless populations. AA degree or Bachelors degree in lieu of experience will be highly considered! Proficient computer skills, including Microsoft Office Suite (Outlook, Word and Excel) Experience working in diverse settings with people across all socio-economic spectrums and a wide variety of personalities and roles - staff, residents, local agencies, contractors, lenders, etc. Position Requirements: Access to reliable personal transportation required, including a DMV record that permits driver to be insured under SHELTER, Inc.’s automobile coverage.
