WAGE RANGE: $24.00-$26.50/hour JOB SUMMARY:  Catholic Charities is seeking a self-motivated, compassionate, trauma-informed, and culturally aware Case Manager II to join our team at Higher Ground - St Paul.  This position provides direct case management services to individuals experiencing homelessness or housing instability, supporting their journey toward stability, recovery, and independence. The Case Manager II partners with clients to assess needs, develop individualized service plans, connect to community resources, and monitor progress. They will serve as an advocate for the client and provide education related to services, community resources, and systemic barriers that impact housing and wellbeing. ESSENTIAL FUNCTIONS: 1. Complete intake and ongoing assessments, prepare individualized service plans, and coordinate services to meet each client’s needs. 2. Support and advocate on behalf of clients to access housing, benefits, healthcare, and other community resources. 3. Assist with required documentation and ensure case files meet funder and licensure requirements. 4. Maintain accurate case notes, service plans, and progress documentation in compliance with Catholic Charities standards. 5. Provide crisis intervention, advocacy, and follow-up as needed to promote client safety and stability. 6. Collaborate with internal teams and external partners to ensure coordinated, trauma-informed care. 7. Educate clients, volunteers, and community partners on available resources and Catholic Charities programs. 8. May transport clients as needed to appointments or community services. 9. Participate in team meetings, supervision, and professional development opportunities. 10. Perform other duties as assigned. MINIMUM QUALIFICATIONS/EXPERTISE: * Bachelor’s degree in human services or related field preferred, with a minimum of two (2) years of experience working with individuals experiencing homelessness, housing instability, or behavioral health needs. * In lieu of degree, three (3) years of directly related experience required. * Demonstrated ability to work respectfully and effectively with people from diverse cultural, economic, and social backgrounds. * Experience with trauma-informed care, motivational interviewing, and harm reduction principles preferred. * Additional training, education, or licensure may be required depending on program needs.
