Create individualized Family Support Plans focused on helping individuals reach their personal best. Support individuals in building meaningful social relationships and engaging in purposeful work. Conduct home visits and maintain consistent, ongoing communication with families. Help families stay connected and reduce the need for out-of-home residential services. Qualifications Experience in human services or a related field. (3 to 5 years experience) Strong communication, planning, and advocacy skills.
