POSITION SUMMARY Under the direction of the Clinical Program Manager will follow established procedures; provides case management and group facilitation, serving at risk youth, parents and families. Provides direct services including but not limited to intakes, screening/assessments, on campus groups and linkage to community resources. Services may also be provided in the community, student's home, and at RCS. Uses program assessment tools and demonstrates cultural competency by effectively working with people differing in value systems, ethnicity, cultural backgrounds, language capabilities, disabilities and professionals of different disciplines, cultural background and value systems. receives and coordinates referrals and engages families in services. Performance of the typical responsibilities and duties requires the ability to perform the job duties, explain to/instruct others, involving job-related matters generally of limited to moderate complexity. TYPICAL RESPONSIBILITES AND DUTIES: • Direct Services & Engagement *  Provide case management, psychoeducation, and group facilitation, serving at risk youth, parents and families. * Conduct intakes, screenings, assessments, and surveys. * Facilitate on-campus groups, evening workshops, and family engagement activities. * Provide crisis intervention and culturally competent support. * Engage families within required timelines using phone calls, letters, and home visits. * Conduct periodic home visits to assess family resources and needs. • Case Management & Coordination * Link and refer children and families to community resources and interagency services. * Monitor progress in collaboration with therapists and program staff. * Support families in navigating complex medical, educational, and social service systems. *  Assist with parenting activities and encourage parent participation. • Documentation & Compliance *  Complete intake paperwork, consent forms, assessments (e.g., Protective Factor Survey, CANS, PSC-35, and CSQ 8), and other clinical care documents. *  Maintain timely and accurate data entry in internal and external databases. * Generate reports (school referrals, outcomes, satisfaction surveys, financial logs) as needed. * Ensure compliance with confidentiality, mandated reporting, and quality improvement standards. • Collaboration & Program Support * Work closely with CPM, call centers, schools, and community partners to coordinate referrals and transitions. * Participate in staff meetings, trainings, PQI activities, and evaluation efforts.  * Support agency events, departmental events, client outreach, and front desk reception when needed. * Provide translation and interpretation support as appropriate. • Core Competencies: * Understands Wraparound Philosophy and integrates principles into their daily job performance. *  Trauma-informed and culturally responsive care. * Strong organizational and documentation skills with attention to deadlines. * Ability to work independently and collaboratively within a team. * Skilled in crisis intervention, engagement, and creative problem-solving. MINIMUM QUALIFICATIONS EDUCATION, TRAINING AND EXPERIENCE: • Associate or bachelor’s degree in social services, human services or related (or 4 years of equivalent years of experience) preferred. • One year experience working in children’s mental health setting emphasizing a family systems perspective desirable. • Must meet the Title 9, Title 19, and Title 22 Medi-Cal Regulations • Experience working in children’s mental health setting emphasizing a family system perspective desirable • Basic skills in crisis intervention, suicide assessment, conflict resolution, and interpersonal communication. • Bilingual/ Bicultural (Spanish/English) and/or other threshold languages preferred. LICENSE(S): • California driver’s license and ability to be insured by RCS insurance provider. KNOWLEDGE AND SKILLS: • Demonstrated ability to provide quality, culturally competent and relevant educational and preventive services to at-risk children and families. • Strong sensitivity to the diverse cultural needs of children and families served. • Proficient oral and written communication skills. • Proficient organizational and time management skills. • Ability to work independently exercising good judgment, make decisions & problem solve. • Ability to work with confidential information required. • Proficiency in Microsoft Office applications. • Must have reliable transportation, valid driver’s license and minimum state required automobile insurance. • Must be available evenings and/or weekends as required to fulfill job responsibilities. SPECIAL REQUIREMENTS: • Must be able to meet and receive a criminal records clearance as required by Title XXII licensing regulations. PHYSICAL AND MENTAL REQUIREMENTS: The following are required in day-to-day performance of the duties of this position: • Driving/Traveling to clients such as school, home and/or community setting. • Sitting, walking, standing, grasping (simple), and visual/auditory acuity are constantly required (over 2/3 of the workday). • Bending, squatting, kneeling, twisting, lifting, reaching, and pushing/pulling are required occasionally (under 1/3 of the workday). • Climbing and crawling are generally not required.
Responsibilities
