Description Case Manager II People Incorporated is seeking Case Manager II's to join our Targeted Case Management St. Louis County team to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. Hours: Full - Time, Monday - Friday, regular business hours Location: St. Louis County, Minnesota - Hybrid Range: $24.00 - $25.35/ Hourly, based on qualifications Hiring Bonus: $1,000 to external candidates People Incorporated Mental Health Services is the largest nonprofit mental health provider in Minnesota. Main Job Duties: Must be able to have onsite presence within the St. Louis County, Minnesota. Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services. Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received. Assessing client needs and ensuring continuity of care. Making ongoing referrals to resources based on assessment and current client need. Maintaining necessary daily records for continuity of service and reports to funding agencies. Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards. Attending client monthly meetings to monitor services. Requirements Requirements for All Case Manager II positions: Valid driver’s license, excellent driving record and current auto insurance Education and Experience Required for Targeted Case Manager II position: Bachelor’s Degree in behavioral science or related field 2,000 hours of supervised experience working with adults with severe and persistent mental illness OR Master’s Degree in behavioral science or related field.
