We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. We prioritize work-life balance and individualized, high-quality care. To support this, our Case Managers carry average caseloads of approximately 30 people served, allowing for meaningful engagement and effective service coordination. Main Job Duties: Must be able to faciliate in-person appointments with clients in Duluth, Minnesota. Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services. Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received. Assessing client needs and ensuring continuity of care. Making ongoing referrals to resources based on assessment and current client need. Maintaining necessary daily records for continuity of service and reports to funding agencies. Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards. Attending client monthly meetings to monitor services. Responsibilities
