Description Case Manager II We are seeking Case Manager II's to work on our Waiver Case Management team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. We value work-life balance and have maneable caseloads that are on average 30! Main Job Duties: Attending bi-annual meetings with clients to monitor services Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers.
