Case Manager Job Responsibilities: Completes assessment of housing and housing related needs Works with Venture House and Apartment Treatment staff, and multi-disciplinary treatment team to establish individualized service plans, which meet the clients’ goals. Establishes and maintains effective relationships with other service providers: e.g. Madison County DSS, SPOA Coordinator, PROS, Care Coordination, etc. Acts as program liaison and primary contact for other service providers as it relates to clients coordination of services. Demonstrates current understanding of the Office of Mental Health regulations, mental health disorders and its treatments. Understands reciprocal relationships between substance abuse and mental health. Assists individuals with linkage and referral to substance use disorder and mental health services where necessary. Responds to and effectively handles emergencies, which may require flexibility of time and area of assignment. Completes all required documentation, in timelines in accordance with program standards. Case Manager Qualifications: Bachelors/Associates and one (1) year experience or HS Diploma and three (3) years related experience. Must have a valid driver's license and access to reliable transportation.
