Main Street Project (MSP) is a community health Centre with a mandate to address the social determinants of health which give rise to issues of addiction, physical and mental health issues and homelessness. MSP uses housing-first and harm reduction principles in the provision of a safe, respectful and accessible place for individuals who are experiencing homelessness or at risk of homelessness in the community. Since 1972, MSP has been providing a range of services to Winnipeg’s most marginalized residents, including emergency shelter and nutrition services, drug and alcohol withdrawal management services, health advocacy and support, case management services, housing support services, food bank services and a mobile community outreach program.

JOB SUMMARY
The case management team is a key connection to outside organizations and the participants trying to access those services. They work with individuals who access MSP services to identify, develop, implement and assess the self-identified goals of participants. Case managers work from a housing first, harm reduction and trauma informed lens to support people to find and maintain housing and to connect to primary care, income, addictions & mental health supports to break down barriers and advocate for their participants. Relationship building is the key function of this team. Referrals into this team come from support staff within MSP or external agencies. They are reviewed by the Program Manager and assigned to a case manager based on the space within a caseload, the acuity of the participant and their immediate needs. Once assigned, the case manager meets regularly with participants within MSP and the community.
