Case Manager – Men’s Recovery Housing Location: Wilmington, DE Schedule: Full-Time Department: Lower-Level Housing About Impact Life Impact Life is a mission-driven nonprofit organization providing recovery-oriented, housing-focused, and whole-person support services to individuals impacted by substance use and mental health challenges. We are committed to dignity, accountability, and long-term stability for the individuals we serve. Position Overview Impact Life is seeking a full-time Case Manager to oversee and provide case management services for residents at two Men’s Recovery Houses located in Wilmington, Delaware. This position is part of our Lower-Level Housing Department and is responsible for supporting residents through individualized case planning, service coordination, and recovery-oriented care. The Case Manager is expected to split their time weekly between both men’s houses, ensuring consistent presence, collaboration with staff, and continuity of care for residents at each location. Key Responsibilities Provide direct case management services to assigned residents across two men’s housing programs Meet regularly with residents to assess needs, support recovery goals, and monitor overall well-being Develop, implement, and update individualized recovery and care plans in collaboration with the treatment team Coordinate referrals and link residents to appropriate internal and external services, including behavioral health, medical, and social supports Maintain accurate and timely documentation, including required case notes and records Participate in recovery planning meetings and interdisciplinary team coordination Conduct ongoing assessments and adjust services based on resident needs and program protocols Facilitate groups and provide individualized education as required Provide crisis intervention and therapeutic de-escalation when needed Maintain confidentiality and deliver services in a non-judgmental, trauma-informed, and person-centered manner Travel between both Wilmington housing locations as part of the regular work schedule Qualifications High School Diploma or GED required Bachelor’s degree in behavioral health or a related field preferred Minimum of two years of experience in behavioral health, housing services, or a related field Experience providing case management services strongly preferred Certification Requirement Applicants must either: Possess an active Certified Peer Recovery Specialist (CPRS) credential, or Meet eligibility requirements to obtain CPRS certification and successfully complete certification within one (1) year of hire.
