Description

CCVT offers the following opportunity for a professional, motivated, and dynamic individual to join our team.

Specific responsibilities include:

  • Provides settlement and trauma informed service to clients including intake and assessment, information and referral, supportive counseling, translation and interpretation and follow-ups
  • Facilitates support groups, including planning group sessions, inviting clients, guest speakers, monitoring and evaluating sessions
  • Perform case management functions.
  • Referrals and Client Accompaniment
  • Collects and maintain records (e.g. clients and community resources) relevant to the program. Input all activities/services provided into OCMS and iCARE or any other data base the agency makes available
  • Provides monthly statistical and narrative reports to Manager, and to conduct evaluations as needed.
  • Attends CCVT staff meetings and professional staff development opportunities deemed necessary and appropriate by the manager
  • Participate in the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services for individuals, groups.
  • Work closely with settlement and trauma counselors and partner organizations.
  • Provide client services at partner agencies and satellite locations, in addition to main office
  • Prepare and deliver public education presentations
  • Prepare reports on volume and effectiveness of client services as per funder requirements
  • Performs outreach to potential clients in the catchment area
  • Liaises with appropriate community agencies in order to link clients to services relevant to their needs

REQUIRED QUALIFICATIONS

  • MSW or BSW, combined with 3-5 yrs of directly related experience in a case management role
  • Recent direct service experience working with Immigrant, refugee and racialized populations addressing substance use and/or mental health problems
  • Excellent engagement and assessment skills
  • Strong brief intervention skills such as solution focused and cognitive behavioral approaches
  • Commitment to working within an anti-oppressive framework and demonstrated understanding of the links between trauma and mental health, emotional health, and substance use problems
  • Excellent knowledge of relevant community resources
  • Ability to work effectively in physical offices or virtual environments
  • Excellent interpersonal and communication skills
  • Strong motivation, time management and organizational skill
    Job Type: Full-time
    Pay: $37.12 per hour
    Expected hours: 35 per week
