Position Details The Case Manager will be responsible for: * Collaborating with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan. * Performing initial intakes, assessments, and timeline summaries. * Providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences. * Providing individual and group treatment and support according to each individual’s/family’s needs. * Supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress. Qualifications The ideal candidate will possess: * Bachelor’s degree in Social Work, Psychology, or another human service-related field required. * Experience in the behavioral health field working with individuals with mental health/developmental disabilities preferred. * Valid PA driver’s license with a clean driving record required. * Candidate required to complete clearances and background checks, at Merakey’s expense.
