Job Overview The Case Manager engages individuals during hospitalization to support discharge planning, ensure a smooth transition back into the community, and provide follow-up services that promote successful community integration. They also conduct assessments within correctional facilities and attend court hearings to advocate for the needs and well-being of the people they serve. Program Info Project for Assistance in Transition from Homelessness (PATH) is an intensive case management service which provide mobile outreach services to the psychiatrically disabled homeless living in the community. This program is designed to reach out to consumers who have not accepted traditional center-based mental health services. This service includes: supportive services, advocacy, housing assistance, mental health services, substance abuse services, and transportation. Each counselor functions as part of a team, and all clients interact with all members of the team. Key Responsibilities: Engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition to the community Provides post-discharge follow-up to support the consumer with community integration Conducts assessments within correctional facilities and attends court hearings to advocate for persons served. Requirements: Minimum requirement of a Bachelor’s Degree in Human Services or Psychology. At least 2 years of experience in mental services preferred Professionalism and a commitment to excellence in care Must have and maintain a valid driver's license with an acceptable driving record.
