Description Title: Case Manager Peer Support Specialist- Assertive Community Treatment (ACT) FSLA: Full-time, Non-Exempt Location: St. Paul, MN Purpose: This position is a member of the Assertive Community Treatment (ACT) team and is responsible for providing direct support and services to individuals with serious mental illness and/or substance use disorder. The Peer Support Specialist will draw on their own experience as a consumer of mental health case management services to provide peer counseling and direct support, as well as model wellness and assist individuals in their recovery. This position collaborates with community partners to ensure client needs are met. The Peer Support Specialist shares their knowledge of the recovery process, symptom management, stigma, and traits needed to be successful with coworkers. Key Responsibilities: Duties and Responsibilities: Provide coaching on independent living skills such as financial, healthy lifestyle, vocation, education, housing, physical, and social skills. Establish and maintain positive working relationships with individuals served, other staff members, community service providers, and families. Provide crisis assessment and intervention to promote in-home and in-community safety among individuals served. Navigate and access available community and housing resources for individuals served. Recognize and identify changes in mental health status to actively support individuals served in stabilizing their mental health. Work independently and recognize when supervision is needed. Demonstrate a commitment to advocacy for the right and ability of people with a severe and persistent mental illness to live in the community, work in competitive jobs, and have access to continuous supports and services. Required Skills/Abilities: Knowledge of mental illness and the needs of people with mental illness. Knowledge of social service and community programs available to assist people with serious and persistent mental illness. Knowledge of psychotropic medications, their uses, and common side effects. Ability to assist clients directly through support, mentoring, advocacy, and coordination to help them obtain the necessities of daily living such as medical/dental care, legal/advocacy resources, financial and money management support, and transportation. Ability to promote an inclusive, diverse, culturally competent, and respectful work environment. Ability to develop and maintain receptive, open, and approachable lines of communication with clients and staff, and provide a friendly environment for visitors. Strong written and verbal communication skills, including composing, proofreading, and editing. Presentation and meeting facilitation skills. Ability to work as a proactive and positive member of a team to deliver quality services. Ability to maintain confidentiality of organization, personnel information, and client data. Ability to use sound judgment and maintain healthy professional boundaries. Analytical and problem-solving skills. Ability to meet deadlines and set priorities. Proficiency with a variety of web-based software and databases, including Microsoft Office Suite. Working Conditions: This position is based in Ramsey County or Dakota County program offices and will have interactions with clients of the program and the public. Some activities may be required in the community. Travel may be required to other program sites. Requires transporting clients in a personal vehicle regularly. This position has the potential to come into contact with bodily fluids, communicable diseases, tobacco smoke, and household pets. Prolonged periods at a desk, working on a computer. Manual dexterity for tasks such as taking notes, using technological devices for documentation, or assisting clients with certain activities if needed. Must be ambulatory and able to negotiate stairways. The ability to lift and carry heavy weights that could be up to 50 pounds. Education and Experience: Required: Meet the requirements to be employed as a Certified Peer Specialist Level 1 in Minnesota: Be at least 21 years old. Have or have had a primary diagnosis of mental illness. Is a current or former recipient of mental health services. Be willing to share their experience of recovery. Successfully completes the Department of Human Services-approved Certified Peer Specialist training and certification exam. Preferred Holds a bachelor's degree in one of the behavioral sciences or related field from an accredited college or university AND has at least 2,000 hours of supervised experience in the delivery of services to persons with mental illness OR Has at least 4,000 hours of supervised experience in the delivery of services to persons with mental illness. Additional Requirements: Must not be actively receiving services from Guild and must not have received services within the last two years. A valid Class ‘D’ Driver’s License, reliable vehicle, satisfactory driving record and necessary insurance. Must be available to work some evening shifts and provide on-call crisis support 24/7 in weekly intervals on rotation. The range reflects the minimum and maximum salary for the position. Compensation will depend on the candidate’s professional experience, skills, and relevant education, training, and certifications.
