Visit us at www.janussc.org You will be responsible for managing high-quality Perinatal Case Management (PCM) to clients that support, enhance, and coordinate the health and other services recommended for mothers and babies. The role includes assessment, goal setting, care coordination, crisis management, coordinating process improvements and executing weekly standing tasks. You'll establish and maintain strong working relationships with peers and partners and provide regular status updates, summarizing issues to key points and actions required. The Case Manager is expected to demonstrate exceptional flexibility, willingness to learn, ability to work effectively on a team, empathic and strengths-based skills, as well as exceptional time management skills. Finally, you'll represent the organization to other internal groups and clients to ensure that Janus’s objectives are well understood. The schedule for this role is Tuesday - Friday 8:15am - 4:45pm and Saturday 6:45am - 3:15pm. Responsibilities: Coordinate, monitor and adjust all aspects of a client's care from intake to discharge Ensure that everyone who delivers care to someone is working toward a common goal Communicate with members of the team and review client health information Screen and assess eligible patients Ensures the provision of comprehensive preventative services to pregnant women and their infants Ensure that a client is engaged with necessary clinical and/or wrap-around services in the community, facilitates smooth transitions, and coordinates the work of multiple caregivers. Evaluate the treatment plans, and facilitate the services needed to ensure all therapies are complete. Coordinates with justice system stakeholders and community partners to ensure best healthcare outcomes for clients. Advocating for patients’ rights and ensuring that they receive appropriate care. Requirements Education & Experience Two or more years of full-time case management experience in substance use disorder treatment programs, or integrated behavioral health programs will be strongly considered as parity with Bachelor’s degree. Bachelor’s degree in behavioral health related field with some experience providing case management or care coordination. Registered or Certified by CADTP or CCAPP, or MFTi or ASW interns are also encouraged to apply.
