Job Details Job Location: Social Services - 778 - Brooklyn , NY 11206 Position Type: Full Time Education Level: 4 Year Degree Salary Range: $24.73 - $30.21 Hourly Job Shift: Day IMPACCT Brooklyn is seeking a full-time Social Services Case Manager to support a diverse caseload of vulnerable and at-risk adults. Key Responsibilities Include: Review all documentation establishing tenants' eligibility for the program and make copies of the files. Create and maintain tenant files. Conduct initial intake and assessment of tenants and tenants' families' needs and periodic reassessments. In collaboration with tenants, prepare initial and periodic revisions of independent living plans, including short-term and long-term tenant goals. Assist tenants in achieving their goals by identifying and connecting them to community resources and making referrals to appropriate services within and outside IMPACCT Brooklyn. Work with tenants to break through barriers to their goals and assist them in advocating for themselves and moving toward self-sufficiency. Conduct group sessions and/or workshops to assist tenants with independent living skills. Conduct Monthly unit inspections of tenants. Recommend and implement strategies to persuade tenants to participate more fully in this process. Monitor tenants' progress toward their goals through regularly scheduled phone calls and/or face-to-face visits, and document it in progress notes. Participate in case conferences. Follow up with tenants and referral organizations to confirm tenant contact and progress on referrals. Provide all required information for weekly/monthly/quarterly/annual reports. May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or (b) have not demonstrated a willingness to participate in the process (lost-to-service); or (c) have become ineligible for services (e.g., moved out of building). May act as tenant liaison/tenant advocate with outside organizations regarding education, healthcare, housing, legal issues, etc. May schedule appointments for tenants with referral organizations. May escort tenants to appointments (educational, medical, social service, etc.) May assist tenants in completing applications for benefits and entitlements and may process applications on tenants' behalf. May evaluate tenants' living conditions through home visits. May prepare marketing materials for the program. Minimum Qualifications Bachelor’s degree in Social Work, Psychology, Mental Health (MSW preferred). Will consider 5 years or more of Case Management experience, Residential setting experience preferred Strong written and verbal communication skills are required Working knowledge of various computer programs, including email, Microsoft Word, and PowerPoint Problem-solving skills and the ability to multitask Residential setting experience preferred Fluency in English is required; proficiency in Spanish is preferred. Must be willing to work a flexible schedule. Physical Demands and Work Environment This job operates in a professional office environment. This role routinely uses standard office equipment, including phones, photocopiers, filing cabinets, and fax machines. IMPACCT Brooklyn adheres to COVID-19 protocols. Noise levels are typically moderate to high, depending on the time of day and client population.
