SUMMARY:

Reentry Columbia is seeking an enthusiastic and committed Case Manager (preferably with lived experience) to work directly with citizens returning from carceral settings or impacted by incarceration/criminal justice system. The case manager will initiate contact with clients, provide referrals to services, maintain thorough and accurate records, and participate in “Warm Night” support groups. Case Manager will be responsible for transporting clients to appointments using their personal vehicle and will work directly with people they support. This is an onsite position serving Greene County NY which will begin on October 1st, 2025.

REQUIRED SKILLS AND ABILITIES:

  • Excellent verbal and written communication skills.
  • Excellent interpersonal and customer service skills.
  • Excellent organizational skills and attention to detail.
  • Ability to function well in a high-paced and at times stressful environment.
  • Proficient with Microsoft Office Suite or related software.
  • Valid and clean NYS Driver’s License required, as well as reliable vehicle to transport clients to appointments and community-based services.
  • Must be able to gain clearance to work in correctional facilities and have no current criminal justice status (pending, parole, probation, etc.).

EDUCATION AND EXPERIENCE:

  • High School Diploma or equivalent (associate degree preferred).
  • Experience in criminal justice, or human services fields
  • Experience working in customer-facing positions.
  • Experience with Substance Use Disorder treatment preferred.
  • Lived experience/credible messenger preferred.

PHYSICAL REQUIREMENTS:

  • Work is performed in an office environment and requires the ability to operate standard office equipment and keyboards. Must have the ability to lift and small carry parcels, packages, and other items, to walk short distances, and drive a vehicle.
  • Required to stand, walk, and sit; talk or hear, both in person and by telephone; use hands to finger, handle or feel objects or controls; reach with hands and arms. Occasionally required to stoop, kneel, bend, crouch and lift up to 25 pounds. Specific vision abilities required by this job include close vision to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.
  • Must be able to operate a motor vehicle.

Responsibilities

JOB RESPONSIBILITIES:

  • Initiate contact with clients in the jail with referrals from jail personnel and with those known to the agency, meet with people referred from parole, and referrals from other agencies. Upon initiating contact with clients, the case manager will interview to determine needs, assist in sending prerelease packet per DOCCS community prep list, create case management plan, and ensure releases are signed by clients.
  • Making referrals and contacting agencies to coordinate services. Work with providers to develop wrap around services.
  • Initiate a file on each client for the purposes of documenting services, maintain documentation on each contact including service updates and service description.
  • Maintain contact with clients to ensure compliance levels and encourage positive actions. Provide transportation support as needed.
  • Provide clients with current information regarding employment, services, stable housing opportunities, training opportunities, health services, banking, etc.
  • Participate in case conferences regularly to discuss concerns and progress with case managers and Executive Director.
  • Work with partners to identify innovative programs, services, and opportunities available to support clients.
  • Facilitate “Warm Night” support groups on an evening to be determined
  • Occasional evening/weekend hours as needed for tabling purposes/ special events
