Case Manager - Full-Time Permanent Registered Practical Nurse, Registered Social Worker, Recreation Therapist, Occupational Therapist (START) Hamilton Apartments Program – Hamilton, ON Salary: $: $52,200- $68,500 Closing Date for Applications: December 19, 2025 Position Description: Reporting to the Program Supervisor and working with the Short-Term Assistive Response Team (START) within Indwell’s Hamilton Apartments Program, as a registered health professional (registered practical nurse, social worker, recreation therapist, occupational therapist or other registered health professional) you will be responsible to provide tenants with short-term case management supports that help them navigate community and social services, connect them to appropriate community referrals, and build capacity to self-manage physical and mental health needs. As a START team member, you will assist in assessing tenant need, care planning, compiling community and social service resources in the Greater Hamilton Area (GHA), coordinating care, documenting, and completing a full referral process to a variety of mental health, addictions, physical health, physical therapy, recovery, vocational therapy, and psychosocial development needs. To learn more, go to: indwell.ca/our-model Hours of Work: 40 hours per week, Monday-Friday. Flexibility is required to meet program needs. Participation in the on-call schedule is required; generally, 1 week every 10-12 weeks is required to support the tenants and each other on the team. The on-call rotation consists of carrying a work cell phone 24/7 for that one week and responding to urgent matters. WHAT YOU WILL BE DOING: Interdisciplinary Team Collaboration - Partner with other program staff from multiple disciplines in a participatory, collaborative, and coordinated approach to shared decision-making around case management and referrals On-site Support - Work within a team in on-site, in-home assessments and interventions to support long-term stable housing through effective holistic supports Short-term Intensive Case Management – Utilizing various asset-based and person-centered frameworks to create and manage care plans with tenants Demonstrated ongoing learning and experience around supporting diverse individuals and communities (i.e., cultural safety, anti-racist and anti-oppressive practice, trauma-informed care, etc.) Health Promotion and Advocacy - Enabling tenants to build capacity to self-manage their health and related concerns, connect tenants to appropriate community and social service supports for health promotion and maintenance, and educate tenants on system navigation and self-advocacy skills and resources within healthcare and community and social services Education, Resource Development, and System Navigation – Compiling up-to-date resources on community and social services in the GHA and surrounding regions, facilitating information sessions and workshops for tenants on various public health and community and social service topics WHAT YOU WILL BRING: Degree in Social Work, Diploma in Nursing (RPN), Recreation Therapy, Occupational Therapy or a related discipline with a focus on mental health, addictions, or disability. Registered with a regulated college of professionals or willing to get registered for that discipline Experience working within a case management framework with individuals with serious mental illness, substance use, or other special needs A background working with individuals who have serious mental illness, addictions, developmental services, or other specialized needs using a person-centered & trauma-informed approach. Knowledge of serious mental illness, addictions, concurrent disorders, dual diagnosis, mental health and legislation. Familiar with the community and social services available in the Hamilton area is required. Working collaboratively with internal & external partners. Partnering with tenants to develop and implement individualized care plans that support health self-management, connection to community and healthcare services, and navigation of social systems. Lead the development of coordinated care planning with internal and external supports. Complete required documentation in a thorough and timely manner with a focus on maintaining and protecting tenant health records. Must demonstrate a warm, sensitive, and caring approach in working with tenants while maintaining appropriate professional boundaries. Having strong initiative, communication, and problem-solving skills with the ability to work independently, be flexible and responsible.
