Description REPORTS TO: Director of Sanctuary Women’s Development Center Norman JOB OBJECTIVE: The Sanctuary Women’s Development Center (SWDC) Case Manager is responsible for the supervision, safety, and care of daily clients, including managing and monitoring day-to-day needs and assistance with tasks such as intake and file work. Implements social/learning activities for adults and children. This position will carry a caseload and has primary responsibility for those assigned, including assessment, goal planning, advocacy, and coordinating services available within the community. The essential job duties include but are not limited to: Effectively perform Intake interviews with clients. Open, maintain, and update client files within established timeframes. Determine if a client meets case management criteria or refer to partner agencies for services Actively engage with the participants respectfully. Active engagement includes: Direct interaction with participants. Provide transportation as needed using an agency vehicle. Answer phones and interact with participants, providing information and resources. Provide administrative and clerical support, including case file maintenance and intake appointments as scheduled. Manage a caseload Support day services as needed. In either a clinical or a field setting, assists clients with challenges such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine the nature and degree of need. Secure information such as medical, psychological, and social factors contributing to the client's situation and complete psychosocial assessment(s). Ensure a service plan is developed interactively with the client to build on identified strengths and address needs. Plan, broker, and monitor needed services through collaboration and coordination with other service providers for unmet needs. Review client progress regularly as part of re-assessment and goal attainment. Assist with scheduling and/or providing group classes on job search, daily living skills, etc. Participate in community organizations/coalitions serving similar populations and needs, such as Community of Care (COC) meetings, as required. Compile records and prepare reports; follow-up to determine the quantity and quality of service provided to the client and the status of the client's case. Actively works with unhoused clients to find affordable, stable housing options. Contact current or future landlords Find housing Provide financial assistance to get into or maintain housing Ancillary Functions: Access and record client and community resource information, manually or using a personal computer, to input and retrieve information. Assist with day services, including processing hygiene pantry requests, laundry services, showers, etc. Prepares reports regarding services as assigned, e.g., accomplishments, monthly statistics, outcome measures, etc. Establish a respectful relationship with persons served while helping the client gain skills and confidence that will empower them to solve their problems. Provide occasional front desk coverage during staffing shortages and employee lunch breaks Performs other duties related to the position as assigned by the supervisor. SUPERVISORY RESPONSIBILITIES: This position may supervise interns and volunteers. Requirements MINIMUM QUALIFICATIONS FOR CONSIDERATION: Bachelor’s degree in social work or human-services related field preferred. Experience working with homeless or other at-risk populations preferred. Experience working with individuals with mental health is a plus. Experience in case management preferred. Bilingual a plus Knowledge, skills, and abilities: Strong, flexible interpersonal skills required to interact effectively with the participants and residents. Demonstrated conflict resolution skills. Excellent organizational and time management skills are required to ensure multiple tasks receive adequate and timely attention. Able to function autonomously and be proactive. PC literate and proficient, specifically in Microsoft applications. SOAR, Bridges Out of Poverty, Mental Health First Aid, civil rights, blood-borne pathogen training, and CPR/SFA training. (Training provided) Strong verbal and written communication and listening skills. Ability to work effectively with all levels of staff and volunteers. Ability to set and maintain healthy boundaries with staff, clients, volunteers, and interns. Ability to adjust communication style to handle diverse situations. Excellent organizational and time management skills to ensure multiple tasks receive adequate and timely attention. Ability to research and utilize community resources. Ability to drive - must have a valid Oklahoma driver’s license. PHYSICAL REQUIREMENTS High energy and stamina to actively engage with participants and residents. Lifting up to 50 pounds Ability to respond quickly to crisis situations. Ability to perform CPR and the Heimlich maneuver once trained. May sit for long periods of time. Small hand tools may be required for light maintenance. May be required to travel by car or plane to business events or other company locations. May also transport clients using agency vehicle. Travel may require prolonged sitting or standing. Accommodation may be made for some of these physical demands for otherwise qualified individuals who require and request such accommodation.
