JOB SUMMARY

The SHINES (Specialized Health and Instruction to Nurture Exceptional Students)Case Manager is responsible for creating and maintaining alignment between the student’s families/caregivers and educational partners. In collaboration with the SHINES Clinician, they gather relevant information from a wide variety of team members to craft, monitor, and update Individual Treatment Plans. This position works closely with the SHINES Special Educator to coordinate student IEP services and community learning opportunities. The SHINES Case Manager is stationed in person on campus. It is common for this position to take lead in small group daily living skill, regulation, and recreational activities. As the primary supervisor of Intensive Needs Behavior Interventionists (INBIs), they are responsible for observation, feedback, and documentation.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Bachelor’s degree in human services or related field, however, bachelor’s degree nearing completion and relevant experience with a strong staff development plan may be considered.
  • Two years’ experience with human services delivery serving children, youth and families, preferred.

SPECIAL POSITION REQUIREMENTS

  • This position is a Mandated Reporter, as such, is required by law to report suspected abuse or neglect to authorities.
  • Must adhere to confidentiality expectations in all forms of communication.
  • Clearance of Criminal Background Check, maintenance required.
  • Valid driver’s license, excellent driving record, and daily access to a safe, reliable, insured vehicle, required.
  • Must be able to work effectively within the structure of the school, keeping Laraway ethics and conduct expectations in mind.
  • Ability to complete and maintain Handle with Care verbal and physical certifications; Trainer certification preferred.
  • Ability to pass First Aid & CPR training and maintain certification, Lifeguard certification preferred.
  • Basic knowledge and ability to use word processing programs as well as standard communication tools such as telephone systems and e-mail.

WORK/PHYSICAL REQUIREMENTS

  • Given the at-risk population served, the position requires ongoing training and preparedness in the execution of physical restraint techniques. This may require running to catch a child.
  • The “Handle with Care” program specifies that individuals with back and neck limitations are likely to be hampered in the execution of the protocols and greatly increases risk of harm to self or student.
  • Ability to engage with youth in a variety of activities ranging from art, music, cooking and physical activities such as walking, hiking, swimming, and other outdoor sports.
  • Ability to lift up to 50 lbs.

Responsibilities
  • Initiate and maintain communication with families/caregivers, service providers from partner agencies, and sending schools regarding treatment offered to the student and services available to the family.
  • In concert with, and under the direction of, the Licensed Independent Clinical Social Worker (LICSW) update Biopsychosocials and develop/update Individualized Plans of Care (IPCs).
  • Monitor and document progress toward IPC goals and maintain corresponding digital documentation.
  • Inform, model, and supervise implementation of a variety of instructional and intervention strategies as outlined in the assigned student’s Positive Behavior Support Plans (PBSPs) and individualized programming protocols.
  • Maintain up-to-date knowledge of each assigned student’s Individualized Education Plan (IEP).
  • In the event of restraint or seclusion, facilitate compliance of Rule 4500 timeframes and associated expectations of communication and documentation, as well as necessary LYFS associated documentation.
