Position Summary
The Case Manager will be responsible for ensuring that program participants receive comprehensive, individualized services to meet their goals. This role requires strong knowledge of community resources and the ability to build and maintain relationships with service providers to secure timely, appropriate referrals.

Key Responsibilities

  • Conduct intake assessments to gather information about participants’ strengths, needs, and goals.
  • Develop individualized, strengths-based service plans.
  • Monitor and evaluate participants’ progress and adjust service plans as needed.
  • Facilitate group sessions to address shared themes and goals.
  • Collaborate with the Program Coordinator to ensure participants receive appropriate and timely services.
  • Establish and maintain relationships with a variety of service providers and state-level agencies.
  • Follow up with participants to confirm successful connections with service providers.
  • Advocate for participants to ensure services are culturally responsive and meet their unique needs.
  • Maintain accurate, up-to-date case records, including process notes and documentation of all interventions and contacts.
  • Participate in regular supervision and ongoing training to enhance skills and knowledge.

Qualifications

  • Ability to commute reliably to Hempstead, NY, or plan to relocate before starting work (required).
  • Experience working with diverse populations in a human services setting strongly preferred.
  • Knowledge of local community resources and state-level services.
  • Excellent organizational, communication, and advocacy skills.

Job Types: Full-time, Part-time
Pay: $28.00 - $35.00 per hour
