Strong organizational, communication, and computer skills are required.   Qualifications: Bachelor’s degree in human services and prior case management experience preferred. A combination of relevant education and experience may be substituted for a degree. If you have a passion for working with seniors or adults with disabilities, then you will fit right in! We are always looking for team members who are driven to quickly learn new skills about all aspects of the benefits and services available to aging Vermonters. Must have reliable transportation and a valid driver's license.   Responsibilities include: * Collaborating with other community partners to create a person-centered plan of care. * Monitoring and reassessing program service plans as required. * Having a working knowledge of services and programs available to older Vermonters and people with disabilities. * Assisting, when necessary, to gain access to existing services such as public benefits, housing, legal services, etc. * Ensuring that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
