Description Bachelor's Degree is required***Pay Rate is 30.00 -34.00 per hour*** SUMMARY OF RESPONSIBILITIES: The Case Manager is responsible for assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human service needs. SPECIFIC RESPONSIBILITIES: In collaboration with the Charge Nurse and MHP, assess the clients physical, social, and emotional needs. · Assist the client to identify services to meet said needs. · Provide resource information and referrals to assist clients in developing community support systems such as developing supportive relationships, self-advocacy, stable housing, education, and employment. · Act as an advocate on behalf of the client in securing identified services and addressing identified needs. · Provide transportation for clients to designated appointments and meetings · Participate in client Treatment Team meetings as indicated. · Report changes in the individual’s condition to the nursing and MHP staff as appropriate. · Is responsible for accurate and timely charting in the EMR using accepted abbreviations and terminology. · Complete documentation as required. · Conduct educational and therapeutic groups as assigned. · Provide motivational counseling and support to residents as indicated. · Provide crisis intervention to clients as needed during crisis periods. · Monitor and supervises residents to ensure their safety within the facility. · Provide backup to floor staff (CPC’s) as needed, including conducting facility rounds. QUALIFICATIONS, EXPERIENCE, EDUCATION and LICENSURE: · A bachelor’s degree in Counseling, Human Services, Social Work, or comparable degree, or five (5) year of Case Management experience in a professional setting. · Active Washington State Agency Affiliated Licensure required. · CPR/First Aid Certified. · Meets health requirements, i.e. – flu vaccination, tdap vaccination, tb testing, and COVID Booster – per licensing requirements. · Valid Washington State Driver’s License required or the equivalent if residence in Washington is new. · Experience using word processing and spreadsheet software. Microsoft Word and Excel preferred.
